Why SMD?

We have a staff coordinator and case manager that will coordinate with the discharge plan team of the hospital. Upon discharged, patient will get dialysis treatment at the clinic instead of having it at the hospital.
We will evaluate with the doctors if the patient needs urgent dialysis or it can wait in the following morning upon admission at the emergency room.
We will promote Dialysis Nurse and Technician tandem to provide integrated and coordinated service to the patients.
